What is Perineural Injection Therapy?

Perineural Injection Therapy (PIT), also known as Neural Prolotherapy, targets the small sensory nerves (called peptidergic nerves) located just beneath the skin. These nerves can become inflamed due to injury, overuse, or chronic conditions, leading to persistent pain, even when deeper tissues appear healed.

PIT uses low concentrations of dextrose injected along these superficial nerves to calm neurogenic inflammation and restore normal nerve signaling. This technique is safe, effective, and well-tolerated—even for patients with long-standing pain.

How does it work?

Perineural therapy involves injections in the layer of tissue located directly under the skin. This treatment focuses on areas of skin above nerve pathways to address pain at its root cause.

Solution of dextrose/saline helps reduce nerve inflammation and block pain-inducing substances. It may also act on ion channels within nerves, supporting healing at a cellular level.

As perineural injections are drug-free and dextrose is a naturally occurring substance in the body, the risk of side effects is minimal.

What to Expect During a Perineural Injection Procedure
 

  1. Initial Consultation: Your doctor will review your medical history, perform a physical exam, and discuss your symptoms to determine if PIT is suitable for your condition.

  2. Preparation: The injection site will be cleaned, and a local anesthetic may be applied to ensure comfort during the procedure.

  3. Ultrasound or Imaging Guidance (if needed): Ultrasound or other imaging techniques may be used to guide the needle to the precise area of nerve irritation or injury.

  4. Injection: A small needle will be used to deliver the Perineural solution around the affected nerve. This stimulates healing and alleviates nerve-related pain.

  5. Post-Injection Care: After the procedure, mild soreness or a temporary increase in pain may occur, but these side effects generally subside quickly. Your doctor will provide aftercare instructions and may recommend rest or light activity.

Treatment Expectations

 

The primary goal of perineural injections is to alleviate pain.
 

Initial treatment may provide relief lasting from 4 hours to 4 days. Subsequent sessions, typically ranging from 4-6 and conducted weekly, aim for a gradual reduction in overall pain levels with the ultimate objective of complete resolution – a zero pain score – and restoration of full function.

Success rates range from 80 to 100%, depending on the specific condition, with a low likelihood of recurrence unless re-injury occurs.

 

  1. How long does it take to see results from Perineural Injection Therapy?
    Many patients experience relief within a few days to weeks, with continued improvement over several months. Depending on the severity of the condition, multiple treatments may be necessary to achieve optimal results.

  2. Are there any side effects?
    PIT is generally safe with minimal side effects. Some patients may experience temporary soreness or swelling at the injection site. These side effects are usually short-lived. Your doctor will review potential risks prior to the procedure.

  3. How many sessions will I need?
    The number of treatments varies based on the condition and individual response. Typically, patients receive a series of injections spaced several weeks apart. Your doctor will create a personalized treatment plan tailored to your needs.
